Description The Osborne Association is looking for a full time Elder Care Manager to work in Brooklyn and Manhattan, as part of the Manhattan Justice Opportunities Consortium (MJOC). Funded by the Manhattan District Attorney's office, the MJOC is a part of New York City's efforts to help justice involved people, their families and friends, build stability and decrease the likelihood of recidivism. Responsibilities: Screens potential clients for the Osborne Association's Elder program enrollment, Orients and describes Elder program objectives, Conducts initial intake assessments, Utilizes assessment tools to identify support service needs, Creates service plans and makes immediate referrals, Maintains contacts with MJOC consortium and other community partners/agencies to refer or obtain additional client services. Requirements Education: Bachelor's Degree required or Master's degree in Criminal Justice, Social Work, Forensic Psychology or a closely related field required preferred. Social Work license (LMSW or LCSW) are highly desirable. Experience: Two to five years of progressively responsible professional experience in a criminal justice environment. Experience with elders who transition from incarceration, including discharge planning, geriatric and cognitive issues/skills, and job skills training is desirable. Description: About the Program: The Elder Reentry Initiative seeks to improve discharge planning and reentry outcomes for older incarcerated individuals by assessing their specialized needs and connecting them with appropriate services upon their release from NYS prisons and jails. We provide in-facility and community-based case management services for participants who are fifty years and older and soon-to-be-released to NYC.About the Position: Within NYS prisons and DOC jails (2 days per week): Perform screenings, including cognitive and geriatric assessments, create comprehensive transitional plans, and provide case management to older individuals who are soon-to-be released. Within NYC community (2-3 days per week): Provide adaptive, strengths-based case management services for older adults reentering the community from incarceration. Cultivate partnerships between service providers within the fields of reentry and aging. Assist peer mentors who work with participants.Requirements: Education: Bachelor's Degree in Social Work, or in a similar field, is strongly preferred; Two to four years of progressively responsible professional work experience in social services for older adults, human services or non-profit agency required.
